Eligibility: To participate in the CDA Presents Anaheim 2025 - TDIC Sweepstakes (the “Sweepstakes”), you must be an active member of the California Dental Association (“CDA”) who is in good standing and licensed to practice dentistry during the Promotion Period. You must also be at least the age of majority in your jurisdiction of primary residence at the time the prize is received and a legal resident of the fifty (50) United States or the District of Columbia. The Dentists Insurance Company (“Sponsor”), its parents, affiliates, subsidiaries, representatives, consultants, contractors, legal counsel, advertising, public relations, promotional, fulfillment, and marketing agencies, and each of their employees, officers, trustees, and directors, and members of their immediate families (spouses, parents, siblings, and children, regardless of where they live) and those living in the same household, whether or not related, are not eligible to enter or win. Participation constitutes participant’s full and unconditional agreement to these Official Rules and to Sponsor’s decisions, which are final and binding in all matters related to the Sweepstakes and which Sponsor may decide in its sole discretion. Winning a prize is contingent upon fulfilling all requirements set forth herein.
Sponsor: The Dentists Insurance Company (“TDIC”), 1201 K Street, 14th Floor, Sacramento, CA 95814. TDIC offers a package of insurance plans for dentists, including professional and dental business liability, commercial property, employment practices liability, workers’ compensation and cyber liability protection.
How to Enter: During the Sweepstakes Period, there are two (2) ways to enter. 1) Visit Booth: During CDA Presents Anaheim 2025 show hours, go to the TDIC booth at the Member Benefit Center (Booth 2238) located in Anaheim Convention Center 800 W Katella Ave, Anaheim, CA 92802 and have your CDA Presents Anaheim 2025 Badge scanned by a sales agent. Scanning your badge automatically enrolls a participant with one (1) entry into the Sweepstakes. 2) Email Entry: To enter without scanning your CDA Presents Anaheim 2025 Badge, send an email to Sponsor at [email protected] RE: CDA Presents Anaheim 2025 - TDIC Sweepstakes. Specific information to be included in entry email: individual’s first and last name, phone number, professional status (Dentist, Dental Office Staff, Dental Student, etc.) and email address.
You must provide all required information to be eligible to enter and win. By entering, you agree that Sponsor may contact you via any social media, including on Facebook, LinkedIn, or Twitter, by sending you a direct message, tagging you, posting to your social media page or naming you on Sponsor’s social media page. Normal internet access, phone, and usage charges imposed by your online or phone service may apply. LIMIT ONE (1) ENTRY PER PERSON, REGARDLESS OF METHOD OF ENTRY. ADDITIONAL ENTRIES MAY BE DISQUALIFIED. Automated or bulk entries or entries submitted by third parties may be disqualified. Entries must be submitted by the participant. Any attempt by any participant to obtain more than the stated number of entries and/or plays by using multiple and/or different identities, forms, registrations, email addresses, logins, or any other methods will disqualify that participant’s plays and/or entries, and that participant and/or entries may be disqualified. Multiple participants are not permitted to share the same email address. Entries specifying an invalid, non-working, or inactive email address will be disqualified. Incomplete, illegible, corrupted, or untimely entries will be disqualified. All entries become the property of Sponsor.
By participating in this Sweepstakes, participant acknowledges and agrees that he or she will clearly and conspicuously disclose that he or she is participating in this Sweepstakes in the entry and any online social sharing interaction that references the Sweepstakes or that is used to obtain entries in connection with this Sweepstakes. Sponsor reserves the right to disqualify any participant who does not adequately disclose his or her participation in this Sweepstakes while engaging in any online social sharing interaction during the Sweepstakes Period.
Selection of Winners: On or about May 29, 2025, Sponsor will select the name of one (1) potential winner of prizes in a random drawing from among all eligible entries received. The odds of winning are based on the number of eligible entries received. The potential winner will be notified by email on or about the day of the drawing. Except where legally prohibited, the potential winner must sign and return, within seven (7) days of being notified, an Affidavit of Eligibility, Liability & Publicity Release and IRS Form W-9 in order to claim his/her prize. If a potential winner cannot be contacted in person or by email after the first attempt to contact him/her, or if he/she fails to sign and return the Affidavit of Eligibility, Liability & Publicity Release, IRS W-9 Form, or any other documentation that Sponsor may require, in its sole discretion, within the required time period (if applicable), or in the event that a potential winner is disqualified for any reason, Sponsor will award the applicable prize to an alternate winner selected by a random drawing from among all remaining eligible entries. Only two (2) alternate drawings will be held, after which the applicable prize will remain un-awarded.
Verification of Potential Winners: ALL POTENTIAL WINNERS ARE SUBJECT TO VERIFICATION BY SPONSOR, WHOSE DECISIONS ARE FINAL AND BINDING IN ALL MATTERS RELATED TO THE SWEEPSTAKES. A PARTICIPANT IS NOT A WINNER OF ANY PRIZE UNLESS AND UNTIL PARTICIPANT’S ELIGIBILITY HAS BEEN VERIFIED AND PARTICIPANT HAS BEEN NOTIFIED THAT VERIFICATION IS COMPLETE.
Prizes: There will be one (1) Prize awarded. The Prize consists of one (1) of the following lodging packages to be selected by the prize winner: (i) seven night stay for two at the luxurious Jepun Villa in Bali (7 nights) (“prize option #1”); or (ii) five nights stay and culinary experience for four in a private residance in Italy's historic city center of Cortona, Italy (5 nights) (“prize option #2”); or (iii) Luxury condo in Costa Rica with Private Concierge for eight for seven nights , (7 nights) (“prize option #3”).

Prize winner will be awarded a vacation to one of the three prize option locations specified above (“Location”). Prize includes standard hotel accommodations (double occup. #nights/# days) at the hotel specified in the selection prize option (“Hotel”). Total ARV of Prize: $5,000.00. ARV of Prize may vary depending on travel dates and room/rate fluctuations.
Prize winner must be able to travel on dates to be determined by Sponsor or risk forfeiting Prize. Exact dates of travel to be determined at Sponsor’s discretion. All airfare, ground transportation, meals, incidentals, and expenses not described herein and incurred by Prize winner will be the sole responsibility of the Prize winner.
Travel must be completed by December 31, 2026. Winner is responsible for obtaining all necessary travel documents, making travel arrangements, complying with all applicable public health and safety protocols (including all requirements adopted by Prize providers), and for obtaining travel insurance (and all other forms of insurance) at his/her option. Winner must comply with all hotel check-in requirements, including, but not limited to, the presentation of a payment card.
Winner’s travel companion must be at time of prize fulfillment the age of majority in the jurisdiction in which s/he resides (and at least eighteen (18) years of age), unless accompanied by a parent or legal guardian, and must complete and return a liability/publicity release and any other documentation required by Sponsor prior to prize award. If travel companion is a minor in his/her state of residence, minor’s parent/legal guardian must complete and return all releases on behalf of minor. Winner and Guest must travel together on the same itinerary. If winner elects to travel or participate in the prize travel without a Guest, no additional compensation will be awarded. If winner elects to travel or participate in prize travel with more than one Guest, such additional persons shall be responsible for any and all costs associated with such travel. Winner and Guest are also responsible for obtaining travel insurance (and all other forms of insurance) at their option and acknowledge that Sponsor has not and will not obtain or provide travel insurance or any other form of insurance. No refund or compensation will be made in the event of the cancellation or delay of any ai except at Sponsor’s discretion. Travel is subject to the terms and conditions set forth in this Sweepstakes, and those set forth by Sponsor’s airline carrier of choice as detailed in the passenger ticket contract. The passenger contracts in use by the airlines or other transportation companies shall constitute the sole contracts with respect to the prize winner’s and his/her Guest’s transportation and such contracts shall be solely between the prize winner, his/her Guest and such transportation companies. Lost, stolen or damaged prize vouchers and airline tickets will not be replaced or exchanged. With the exception of stopovers approved in advance at Sponsor’s sole discretion, no stopovers are permitted on tickets issued as part of the prize. If a stopover occurs, the prize travel will terminate and winner and his/her Guest will be responsible for the full fare which will be charged from the stopover point for the remaining vacation segment(s), including the return. Sponsor shall not be responsible for any cancellations, delays, diversions or substitutions or any act or omissions whatsoever by any air carrier(s) or other transportation companies or any other persons providing any of these services to passengers including any results thereof such as changes in services or accommodations necessitated by same. Airline tickets issued in connection with the prize may not be eligible for frequent flyer miles or upgrades. Winner and Guest must comply with all hotel check-in requirements, including, but not limited to, the presentation of a payment card. Air transportation and hotel accommodations must be taken together and cannot be taken separately.
In the event winner engages in behavior during travel that (as determined by Sponsor) is obnoxious or threatening, illegal, or that is intended to threaten or harass any other person, or that in any way disparages or adversely affects the reputation, image, and/or customer goodwill of Sponsor or any of Sponsor’s services, products, trademarks, service marks, or logos, Sponsor reserves the right to terminate any prizes, to terminate the travel early, to terminate the lodging, and send winner home (at his/her own expense for amending existing tickets, reservations, and/or related prize contracts) with no further obligations or compensation whatsoever to winner (which may, in Sponsor’s discretion, result in such winner’s disqualification from the Sweepstakes and forfeiture of any (and/or all) prize(s). In the event winner engages in behavior during travel that (as determined by Sponsor) is illegal, tortious, or subjects winner to arrest or detention, Sponsor shall have no obligation to pay any damages, fees, fines, judgments or other costs or expenses of any kind whatsoever incurred by winner as a result of such conduct. SPONSOR SHALL HAVE NO LIABILITY FOR ANY PERSONAL INJURIES, DEATH, PROPERTY DAMAGE, OR OTHER DAMAGES OR EXPENSES RESULTING FROM OR ARISING OUT OF ANY TRAVEL RELATED TO THE PRIZE OR ANY OTHER ASPECT OF PRIZE WINNER’S ACCEPTANCE OR USE OF THE PRIZE.
Prize Restrictions: Any and all applicable federal, state, and local taxes and all fees and expenses related to acceptance and use of prize not specifically stated herein, including but not limited to, change of air travel fees, ground transportation, automobile insurance, meals, incidentals, passenger tariffs or duties, surcharges, service charges or facility charges, personal charges at lodging, security fees and/or other expenses, are the responsibility solely of winner. Prize cannot be substituted, assigned, transferred, or redeemed for cash; however, Sponsor reserves the right to make equivalent prize substitutions. Sponsor will not replace any lost or stolen prizes or components of a prize. Prize cannot be used in conjunction with any other promotion or offer. Only the number of prizes stated in these Official Rules is available to be won in the Sweepstakes. If, by reason of a print or other error, more prizes are claimed than the number set forth in these Official Rules, prize winner(s) will be selected in accordance with the winner selection method described above from among all eligible claimants making purportedly valid claims to award the advertised number of prizes available.
General Conditions: WARNING: ANY ATTEMPT BY A PARTICIPANT OR ANY OTHER INDIVIDUAL TO DELIBERATELY DAMAGE ANY WEBSITE ASSOCIATED WITH THIS SWEEPSTAKES OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAW, AND, SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO PROSECUTE AND SEEK DAMAGES FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW. Sponsor’s failure to enforce any term of these Official Rules shall not constitute a waiver of that provision. Neither Sponsor nor anyone acting on its behalf will enter into any communications with any participant regarding this Sweepstakes, except as expressly set forth in these Official Rules. Sponsor reserves the right at its sole discretion to disqualify any individual it finds to be attempting to tamper with or undermine the entry process, any website associated with the Sweepstakes, and/or the legitimate operation of the Sweepstakes; to violate the Official Rules; or to act in an unethical, unprofessional, obnoxious, unruly, or disruptive manner or with the intent to annoy, abuse, threaten, or harass any other person. Sponsor and its agents are not responsible for (1) any incorrect or inaccurate information, whether caused by participants, printing errors, or by any of the equipment or programming associated with or utilized in the Sweepstakes; (2) technical failures of any kind, including, but not limited to malfunctions, interruptions, or disconnections in phone lines or network hardware or software; (3) unauthorized human intervention in any part of the entry process or the Sweepstakes; (4) technical or human error which may occur in the administration of the Sweepstakes or the processing of entries; or (5) any injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from participant’s participation in the Sweepstakes or receipt or use or misuse of any prize. If for any reason a participant’s entry is confirmed to have been erroneously deleted, lost, or otherwise destroyed or corrupted, participant’s sole remedy is another entry in the Sweepstakes, provided that if it is not possible to award another entry due to discontinuance of the Sweepstakes, or any part of it, for any reason, Sponsor, at its discretion, may elect to hold a random drawing from among all eligible entries received up to the date of discontinuance for the prize offered herein. If, for any reason, the Sweepstakes is not capable of running as planned due to causes beyond the control of Sponsor, which corrupt or affect the operation, administration, security, fairness, integrity, or proper conduct of this Sweepstakes, Sponsor may, in its sole discretion, void any suspect entries and (a) cancel or terminate the Sweepstakes (or any portion thereof) and award the offered prize at random from among the eligible, non-suspect entries received up to the time of the impairment or (b) modify or suspend the Sweepstakes to address the impairment and then resume the Sweepstakes in a manner that best conforms to the spirit of these Official Rules.

Disputes: This Sweepstakes is governed by the laws of the United States. Except where prohibited, participant agrees that: (1) any and all disputes, claims and causes of action arising out of or connected with this Sweepstakes or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by the United States District Court for the Eastern District of California or the appropriate State Court located in Sacramento County, California; (2) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering this Sweepstakes, but in no event attorneys’ fees; and (3) under no circumstances will participant be permitted to obtain awards for, and participant hereby waives all rights to claim, indirect, punitive, incidental and consequential damages and any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased. Sponsor and Sponsor’s agents, affiliates, subsidiaries, representatives or service providers may use participants’ and/or referrals’ personal information submitted with entry for purposes of prize fulfillment and/or to update the membership database maintained by Sponsor.
